Terms and Conditions

Complete and sign our copy of the Terms & Conditions - This shows that you agree to the terms under which you are using us to reclaim your money.

It is very important to read this document. Once you have read it, complete the bottom section by signing it (1), printing your name and adding the date. The second box (2) is only necessary if you are putting in a joint claim, in which case it must be signed by the other claimant.

Your version of the Terms & Conditions is identical except that it has no signature box. This is for you to keep.

Form of Authority and Engagement

Complete and sign the Form of Authority & Engagement – this instructs us to act on your behalf and authorises the financial institutions to deal with us.

If you have received more than one copy of this form, it will be because you are looking to make multiple claims. We need one form for each claim.

This form comes with some information already completed after we spoke to you on the phone. Please check this information to make sure we got it right.

The top of the form has the details you gave us for you (and the joint claimant if applicable, if not then the Customer 2 section will be left blank). You may need to add an address if your previous address was the one registered with the lender and it has not already been entered.

The bottom of the form is to sign to show that you agree to the terms. Both parties should sign here, if this is a joint claim. Also include any previous names and previous signatures either you (or the joint claimant if there is one) have used.

Additional Information

Complete the Additional Information form – this will assist us in progressing your claim as quickly as possible.

This form is for you to add any other information you think may help.

The top section is for the details of the finance you think you had PPI on.

Add:

  • The name of the lender (bank/building society etc.)
  • The type of finance - whether it was a credit card, loan, mortgage etc.
  • Any account or agreement numbers you may have for the finance
  • The dates you had the loan or finance for (even an approximation will help)

The next section is for any previous addresses you may have lived at during the period of the finance.

The third box is for you to add any other information you may have which may help us speed up your claim.

We offer many other financial services and by ticking the boxes under 'How else can we help?' you will receive information on these.

Finally, don't forget to complete the contact section with your additional contact details, where you can specify your preferred method of contact and the best time of day to get in touch.

Return these 3 items (or more if you have more than one Form of Authority and Engagement) in the prepaid envelope along with any other documentation relating to your claim that you might have.

Don't forget

If you have moved house since you took out the agreement please supply your old address. If you have changed your name by marriage or deed poll please try to include evidence of your former name/s.

If it’s a joint claim, it’s important that both partners sign all the documentation.
